How to Obtain Escalation Sigils:
Complete Strongrooms on Torment I difficulty or higher.
Open the Horadric Caches awarded for completion — these have a chance to drop Escalation Sigils.
Use the sigil to unlock the first dungeon of an Escalating Nightmare sequence.
Once you activate a sigil, there’s no turning back. You’re committed to finishing all three dungeons in one sitting. If you quit or fail, your run ends, and the sigil is consumed.
This system emphasizes commitment and endurance — just reaching the final boss is an accomplishment in itself.
Affix Stacking: The Real Nightmare
Nightmare Dungeons already test players with randomized affixes that change the flow of combat. But Escalating Nightmares transform these modifiers into a cumulative gauntlet of agony. Each time you move to the next dungeon, new affixes are added on top of existing ones.
Let’s say you start with:
Volcanic (explosive ground effects)
Drifting Shade (low visibility)
The second dungeon might add:
Stormbane’s Wrath (roaming electrical storms)
By the third dungeon, expect another two or more affixes added to the pile. You could end up dealing with:

End of the Line: Exalted Astaroth and The Amalgam
After navigating the dungeon gauntlet, players face off against a reworked version of Astaroth — now known as Exalted Astaroth — alongside his resurrected demon-hound mount, the Amalgam. This is not the same fight from the campaign. This is a two-boss battle, and both enemies are lethal in their own right.
